The Direct Coombs Test (also referred to as the Direct Antiglobulin Test or DAT) determines if your immune system is attacking your red blood cells (RBCs). The DAT looks for antibodies or complement proteins that are stuck to your RBCs, causing them to undergo hemolysis (or breakdown) and leading to conditions like anemia. Performing a DAT is of value to identify if you have "autoimmune hemolytic anemia," "hemolytic disease of the newborn," or "transfusion reactions" so that your doctors can manage the underlying reason for your symptoms appropriately.
Process
- A small amount of blood is collected, typically from your vein.
- The red blood cells are washed and are mixed with a Coombs reagent.
- The result is positive if there is agglutination (clumping of cells).
When to Get Tested
You should consider getting tested if you have anemia, which could present with symptoms like fatigue, jaundice, and dark urine, especially if your doctor suspects an immune cause. The test is often performed after transfusions, or for women in pregnancy who are at risk of hemolytic disease of the newborn.

Understanding Your Results
A positive test means your RBCs are coated with antibodies or complement indicating immune-mediated hemolysis. A negative test indicates no immune attack on the RBCs. Your doctor may order other tests to determine the cause.
