The Ziehl-Neelsen stain, also called acid-fast stain, is a laboratory test that is used to identify certain types of bacteria, particularly Mycobacterium tuberculosis, which causes tuberculosis (TB). The Ziehl-Neelsen stain is important because it can help diagnose TB and other infections as early as possible so that treatment can begin and transmission can be curtailed.
Process
- The specimen (ex. sputum) is placed on a slide and heat-fixed.
- The slide is stained with a red dye (carbol fuchsin) and then washed with acid-alcohol.
- A second stain (blue or green) is placed on the slide and viewed microscopically.
When to get tested
Individuals with coughs that are persistent, febrile, nocturnal sweats, weight loss, and especially individuals who are at higher risk like the HIV/AIDS population, should consider this test. It is also suggested in those who you think may have TB or leprosy as well.
Test Preparation
No special preparation is needed. For sputum samples, early morning collection after rinsing the mouth is ideal.
Sample Type
Common samples include sputum, bronchial washings, gastric aspirates, or tissue biopsies.
Understanding Your Results
A positive test shows red acid-fast bacteria, indicating that an active infection is present. A negative test does not completely rule out disease so you may want to consider additional tests, such as cultures or PCR tests, for confirmation and to plan further treatment.
